KEY FINDINGS

### Moderate Aortic Stenosis (AS) With At-Risk Features

Moderate AS patients outnumbered severe AS by nearly 2:1

PROSPECTIVE

### Phase 2

Participate in ACTIVATE AS

It’s the CardioCare® platform’s first prospective clinical study. ACTIVATE AS is designed to use CardioCare to automatically identify patients for and track events and outcomes using EMR and echo data. Enrollment is expected to begin in 2027.

RETROSPECTIVE

### Phase 1

78.7%
of moderate AS patients had at least one at-risk feature

![ACTIVATE\_chart\_two\_year\_outcomes-transparent](https://info.egnitehealth.com/hs-fs/hubfs/ACTIVATE_chart_two_year_outcomes-transparent.png?width=500&height=317&name=ACTIVATE_chart_two_year_outcomes-transparent.png "ACTIVATE_chart_two_year_outcomes-transparent")

On the last echocardiogram before the event:

### 90%

of deaths

### 86%

of hospitalizations
with heart failure

among moderate AS patients occurred while still moderate, without evidence of progression to severe AS

| **AT-RISK FEATURES** | |
| --- | --- |
| Valve-related symptoms | LVEF below 60% |
| Grade 2 or higher diastolic dysfunction | Low stroke volume index |
| Atrial fibrillation | Elevated natriuretic peptides |

CLINICAL STUDY

### More about ACTIVATE AS

“Current guidelines recommend echocardiographic surveillance every one to two years for moderate AS, with valve replacement deferred until severe criteria are met. Yet in a recent large real-world study of over 53,000 AS patients, most deaths and hospitalizations with heart failure occurred while patients were still diagnosed as moderate.

ACTIVATE AS will identify and follow these patients prospectively to help us understand the natural history of disease. We welcome heart programs who want to play a role shaping the future of moderate AS patient care to reach out to learn more about participating in the study.”

#### ACTIVATE AS

#### 1 Identify

CardioCare identifies patients with moderate AS across participating sites, grouped by disease phenotype.

#### 2 Evaluate

Enrolled subjects are recalled for heart team evaluation, including echocardiography assessed by a core laboratory, natriuretic peptide testing, and CT aortic valve calcium scoring.

#### 3 Follow

Every six months for up to two years or until patients develop severe AS or undergo aortic valve replacement.

#### 4 Analyze

egnite data scientists will collaborate with study investigators and lead efforts on analyzing findings.
